Little Sky Eyes Grant Application

Before applying, please:

  1. Please read the Grant Guidelines to ensure your child and funding request meet the eligibility criteria.
  2. Obtain a current quote from an Australian supplier in Australian Dollars (AUD) for the item or service you are requesting.
  3. Gather supporting documentation, including:
    • Medical documentation confirming diagnosis
    • Letter of recommendation or support
    • NDIS plan and/or funding outcome (if applicable)
    • Product or service quote
  4. Submit all supporting documentation before the grant round closes.
  5. Allow enough time to complete the application, as progress cannot currently be saved.

Information provided in your application will only be used to assess and administer grant funding. Personal information, photographs, or identifying details will never be published or shared without the consent of the applicant, parent, or guardian, unless required by law.

Please attach a letter of recommendation report from one of the following qualified professional on your Childs care team. Occupational Therapist, Speech Therapist, Physiotherapist, Neurologist, Paediatrician.
